Formation for Spiritual Companions (FSC)

Formation for Spiritual Companions (FSC) is designed to be purchased and used by a faith community. A covenant agreement that accompanies the packet gives the community permission to use FSC at one geographic location from that time on.

Formation for Spiritual Companions costs $495 plus shipping and handling (U.S. currency). To recoup this start-up cost, plus the cost of running the program, most groups charge students a modest tuition.

The FSC covenant agreement also states that after the first time Formation for Spiritual Companions is offered, a renewal fee of $100 is due each subsequent time the one-semester program is presented.

Formation for Healing Ministry (FHM)

The Formation for Healing Ministry Program costs $650.00(US), plus a royalty fee of $25.00(US) for each student handbook that is copied. (each student will need his or her own handbook), plus shipping and handling.

The cost of the program includes a period of telephone, FAX, or e-mail support from experienced ICM staff and the following materials:

• A Coordinator's Manual
• Reproducible Student Handbook (royalty fee)
• A Coordinator's Workbook
• Set of 40 Audiotapes
• A Videotape
